Wife Wants Breast Augmentation Surgery.....Opinions Needed
« on: January 09, 2004, 07:35:08 PM »
I like large-breasted women and surprise, surprise, my wife wants to have BA surgery! Now she is doing this for me but for her TOO! In fact, I'm a bit nervous. She wants to show her new and improved breasts to everyone, including web sites. Yikes! Currently she has a C cup but they are sagging due to having three children and breastfeeding them. She wants to go to a DD size. But she does have questions and I'm sure some of you on here have personal experience with this issue so here goes the questions:

1) She's decided somewhere between 500 and 700 cc implants. Any opinions on this? She does not want to have the surgery done if there is only a slight improvement. She wants NOTICABLY larger breasts. But she doesn't want to be unnatural looking either.

2) In doing research it seems to me that many surgeons are pretty conservative. She doesn't want to fall for the line that large implants aren't for her and she'd be better off with 350 cc implants so that she has a more "natural" look. She wants big boobies, damn it! How should she approach this issue during consultation? Did anyone have a conservative surgeon here? Any questions she should ask at the consultation? She doesn't want to come across as too pushy but she feels, and rightly so, that to pay thousands for surgery that will only make a modest improvement is NOT worth it! Hear! Hear!

If anyone has any ideas, thoughts, opinions, I'd love to hear them. I'm pretty pumped about this and so is she.

I'll let someone with more experience (Ms. Charm, are you out there?) answer number 1 but number two is straightforward.  You and your wife are going to be paying lots of money for someone to do a surgical procedure.  Your focus should be to get exactly what you both want in the safest manner possible.

Part A: Getting exactly what you want.  Many PS's have the ability to computer morph what the results will be, they should also have falsies or padded bras for her to wear.  Do it, let her experience what the results will be until she finds the "perfect" set.  Then, get that.  Don't be afraid of being pushy, the surgeon isn't doing you a favor, they're performing a service and you should get exactly what you want.  Don't compromise if you don't have to, your wife will be living with the results.  Don't even think of scheduling a surgery date until your wife and you are both happy with what you will be getting, a breast augmentation that isn't quite what you wanted will always be a bit of a disappointment.

Part B: Getting a good job done.  First off, you need to be sure that you're getting a good surgeon.  Accreditation is a good thing but only ensures a basic level of competence.  Check with your various state agencies for outstanding complaints and lawsuits.  Ask to speak to former patients and ask them frankly if they're happy with what they had, what kind of complications that they had.  These references should be from women who've had identical procedures done, ask to see before and after pictures of several patients.  Also, cosmetic surgery is far more of an artistic endeavor than most other surgery.  Breast augmentation is not just cramming a fluid sack under the skin, there's an artistry to dissecting perfectly and orienting the bag so that you get the most aesthetic results.  That's why you need to see unclothed pictures of patients, to see how good the results look.  The better the surgeon, the more that they'll cost.  Buy the very best that you can afford, cheaper almost always means less experienced, less careful, or less capable.

Basically, figure out exactly what you want and then put the prospective surgeon through the wringer.  Your wife's appearance and life will be in your surgeon's hands.  BTW, it will also be in his/her OR staff' hands, ask to talk with them, make sure that they have experience and that his/her team has some longevity.  A team with high turnover may indicate a less capable team.

Thanks Matt for your advice, its very helpful. She's in the beginning stages right now. We've been approved for financing. She wants BOTH a lift and an augmentation so I'm not sure what that will run financially. We live in the Pacific NW. I've sent her some pictures of women who went from a C to a DD (just like she wants to do). I must say they all looked very good even at the higher end at 700 ccs. The woman looked large but natural looking. Her main concern in doing this is that a very high percentage of woman wish they had gone BIGGER after they have had the surgery. I'm paying for the surgery but she doesn't want to go through with this if the differences are decent but marginable. But I guess you can take it too far too and she ends up with a DDD size instead. (not that I'd complain but she does have her upper limits). So implant size is kind of a tightrope walk, go too small or too large and she'll regret it. I hear there are some photo programs that can morph the breasts to see them at different sizes. Is this true? I've also heard of the "rice test" too. Still would like some more feedback from you good folks.

Actually, the implant size for a first breast augmentation surgery is sometimes limited by the degree of breast skin elasticity.  However, since the subject of a "lift" was discussed, I would assume that your girlfriend has more than enough skin to accomodate whatever size she chooses.

In regard to implant cc rating, several other factors need to be considered i.e. the "profile" of the implant (or diameter to forward projection ratio), the size of your girlfriend's rib cage etc.  A 700cc rated "high profile" implant made by one company might result in DDD cups but leave a 2" gap between breasts, whereas a 700cc rated "standard round" implant might result in not quite DD cups but killer cleavage.

Also, "lift" surgeries ALWAYS involve the creation of scarring and sometimes result in permanent loss of nerve sensation.  IMHO your girlfriend would be much better off to skip the "lift", avoid the scarring and possible nerve damage, and if necessary go with slightly bigger implants to tighten up her loose breast skin.

In the grand scheme of things of the big boob universe, there isn't a tremendous amount of obvious size difference between 500cc and 700cc or 1000cc implants compared to the 2000cc or 3000cc or 4000cc rated implants that you'll see on BEA girls.  The major difference is in fact a larger DIAMETER rather than more forward projection.

Unless your girlfriend is 5' 1" tall, wears a size 30 bra and has a tiny rib cage, getting implants anywhere in the 500-1000cc size range is not going to result in breasts which "hit people over the head" when she walks down the street unless she deliberately dresses to advertise them!  Now if she was contemplating 2000cc implants like Vixens, she'd definitely be "hitting guys over the head" in a figurative sense.  And with 4000cc implants like Kiesha's, she'd probably be "hitting guys over the head" - literally LOL!

Thank you Melanie, your post was very helpful in understanding all of this. My wife and I read some info and kept coming across "hp" a lot but now we know what that means. As for her size, she's of average height (5'5") and of medium build. She has a lot of skin to work with. She's breastfed our youngest [censored] and her breasts aren't the same size anymore :( So I'm thinking with her build and what she already has, she could handle a larger size implant around 700 ccs. She's very excited about the surgery and will schedule a consultation with a doc on Monday. I also agree with you Melanie that she does NOT need a lift. I have seen many women similar to my wife ( in their 30's, breastfed their [censored] ren) who looked great after JUST breast augmentation surgery. I know they have to do more extensive work in a breast lift rather than a breast augmentation so I will pass this all along to her.

I don't want to make too long of a post. My wife can email you more if you like. But you and your wife's points are very valid.
My wife has had 475cc smooth/round/saline/over implants for over 4 years now. She is about 5'5" tall, about 135 Lbs and was previously a 34B. She is now a good 34D/DD. But we had a conservative surgeon and she went in wanting 600cc minimum, but we didn't push the surgeon hard enough and when the surgeon mentioned only about 350, we only pushed for 400cc implants filled to 475cc. And about a year later when the implants settled, my wife was very dissapointed that we didn't stand firm on getting 600cc implants. So now we are waiting to see if we will have kids, and she will go larger later. So you do need to be direct and firm. But at the same time, that may also mean that you will need to be patient; as some surgeons may turn you down because of that. But if she is a C, and wants to be a good DD, then most likely you will want to be firm on at least 500cc. Just remember that when they have her use the "sizer" implants during consultation, that what ever they tell you, it won't look nearly as large as they do during consultation. I think they try to scare women into smaller sizes. Probably to have them come back in the future for a 2nd BA. Just be firm on what you want. She should basically come out of surgury thinking that her breasts look huge, because within a year afterwards, she will most likely feel they are either just right, or not large enough. Good luck on your consultation(s).

Thanks Xenon for the heads-up. I think we need to stand firm with the doc when the consultation comes. She wants me to go with her. I have heard over and over again these two things about breast augmentation surgery in the little research I've done so far: 1) many docs are very conservative and women can buckle under the pressure of lowering the size of their implants, we both agree that large breasts look fantastic but some of these docs are just set in their own ways, I'll be there with her and if he won't do a large implant (+500 cc's) she will just not have it done by him. So I guess he'll be outnumbered during the consultation.
2) MOST women wish they had gone larger after their augmentation surgery. This is the situation that my wife DOES NOT want to get into. We are going to pay $5000+ for this procedure and she does not want to have second doubts.
I guess the two are related, a pushy conservative doc implanting smaller-than-wanted implants results in situation #2 above. Thanks Xenon for sharing your story. I guess the lesson to be learned is for the woman to stand firm and get the size implants she wants. Also, its probably good to go to a couple of consultations. In looking at this doc's website he seems to be one of the conservative ones. Most of his patients on his site got around 350 cc implants. Now maybe its just the ones he chose to post on his site and he goes larger. I guess we won't know until the consultation.

One word of caution; whatever size you and your wife determine is what she really wants, make sure that the surgeon has a decent amount of experience with that identical implant filled to whatever level you want.  You want someone with experience in making that exact implant look good.  Ask to see post-op photos of women who've had that implant put in to make sure that you like what the probable results will be.

IMHO, if it's not a life-threatening condition, take the time to fully evaluate your surgeon.  Most people agonize far more about the car or house that they buy than the surgical team that will cut them open.

That's nuts!

At 5'5" and with an average ribcage, her best bet is probably to go with standard round implants.  I also agree with your assessment that she's much better off sidestepping the potential negative scarring and nerve damage aspects of a lift surgery (not to mention saving the extra cost) the first time around, and instead go with slightly larger implants i.e. 800cc to firm up her breasts without having to remove breast skin.  I can pretty well guarantee you that even with overfilled 800cc rated implants, with her medium sized frame she's not going to look like "2 boobs on a stick" - which I get the feeling is the look she's trying to avoid.  In the worst case, there's always the option of going for the lift surgery (with or without an implant size change) in the future, and for about the same total cost of a combination lift surgery with implants the first time out.

As far as many plastic surgeons being conservative in regard to implant size, IMHO this stems from three causes - #1 fear of malpractice suits from housewives ("your honor, he turned me into a BIMBO"), #2 lack of experience with implants in the 600-800cc+ size range, and most importantly #3 the fact that doing a first surgery with 500cc implants followed by a second surgery with 800cc implants results in twice as much income for the plastic surgeon as simply putting in the 800cc implants in the first place!

The most effective technique is getting a plastic surgeon to completely understand that his own subjective recommendation of what he thinks is a good size for your wife is IRRELEVANT, that you wife knows exactly what she wants size wise and is willing to accept any consequences of her decision, and that she would prefer that any size errors occur on the side of being too large rather than winding up too small.  You may in fact have to consult with a few doctors until you find one who is comfortable putting in the implant size that your wife wants, and who is willing to forego the extra fees associated with a lift procedure which your wife would dearly like to avoid.

Thanks everyone for the helpful advice. I really appreciate it. I agree with pretty much what has been said so far. I think it is wise to err on the side of having implants that are a little too big than small. Now as for the breast lift, this is my wife's desire but in looking at pics of the procedure, I'm not so sure its wise. I also like the advice of seeing women who had the procedure done with the same size implants as my wife wants to have. Good point, maybe the doc isn't as skilled with larger implants. I went back to this doc's website and the BIGGEST implants were 350 cc. Many were under 300 ccs. Now the women looked fine and each looked better after the surgery so the guy definitely looks like he has some skill. But I'm a bit concerned of why he didn't highlight higher size implants unlike some other docs. Maybe this is his comfort zone but its too early to tell at this time. Its also an interesting point that maybe docs want to place smaller implants to increase their business down the road. Hmmmmm...... it does make you think! I am going to share all this great feedback with her and see what she thinks.

Why yes both my wife and I have seen the 700 cc's you are referring to. We both agree that the woman looks very good and natural too. My wife appreciates all the feedback that you kind folks have provided. BA surgery is NOT to be taken lightly and we want to be as informed as possible before making any final decisions. I do know this though in talking with her today:

1) a MINIMUM of 600 cc's smooth round implants, she would like in the 700 cc range but she's willing to negotiate with the doc and take his expertise into account too, but it sounds like she already has the mindset that the surgeon will try and talk her lower, I don't know if this is a good mind set to go into, I think she should stick to her guns and go for 700 cc's, but she does want to take into consideration the doc's opinions too. But she walks if he shows any hesitancy on 600+ cc's implants. We will find someone else.

2) She is waffling back and forth between getting the breast lift as well as the augmentation. She's very concerned, and rightly so, that she wants large firm perky breasts. She does NOT want large sagging breasts and thinks that an augmentation alone will not be enough. Evidently there is a less intrusive breast lift technique out there but she is quite scared about doing a breast lift. I totally understand her position. If she doesn't go with the breast lift and the augmentation alone doesn't send her breasts northward, what's the point?

3) We are going to go to a few consultations in the Portland, Oregon area. There's one doc that looks very experienced and is willing to do what my wife WANTS. However he seems to prefer placing the implants OVER the muscle tissue and not under the muscle tissue like most docs prefer. This just seems a bit strange to us. So one doc looks skittish about large implants, the other goes against what most docs do nowadays. There are still other docs out there but these two gentlemen look highly qualified. I guess we won't know until the consultation stages.

actually, to achieve the best effect in regard to firm perky implanted breasts from implants alone (without the lift surgery to remove breast skin), the OVER the muscle positioning of saline implants overfilled 20% beyond their rated size seems to achieve the best results.  If the implants are placed under the muscle, you then have to contend with any remaining loose natural breast tissue between the muscle layer and the skin.  With over the muscle placement, the implant is free to stretch out the breast skin and tighten it back up.

If you check out Visitor #12  you'll see how effective this technique can be.
