I don't think that's a problem limited to guys with oversized dicks. I know several guys who've complained of pain with that position. I think it's actually to do with the variable of how far up the penis goes during erection. Some penises, when erect, get fully hard but only rise to half-mast (pointing straight out), and they can move it around in any way they please, and some when erect point straight up, parallel with the body. For those, as far as I understand, they can be bent down somewhere around as FAR as half-mast, and anything more (and sometimes even that far) can be incredibly painful. I'm tempted to say this has something to do with the supporting ligaments.  The same ligaments which get severed during one of the penis enlargement surgeries.  The surgery allows the penis to swing farther from the body, but the trade-off is not being able to get the mast to the same height as previously.
