I don't know how useful this is, but there was someone on this forum from some years back who went by the nickname of DrGirlfriend or Dr_Girlfriend or Dr Girlfriend who was based out of San Francisco who claimed to be getting up to 5000cc implants from someone in the area. You might be able to search out that name using the forum's built-in search, or you could try using Google by searching on "Dr_Girlfriend site:forum.bearchive.com" (and of course the other variants of "DrGirlfriend" and "Dr Girlfriend").
I *think* the surgeon might have been Dr Foster, but I'm going from what I think I vaguely was able to piece together from connecting the dots some time back.
There's also Dr Yermian, Dr Domanskis (and you don't need to go to Italy to go gigantic, merely if you want to try and get larger implants in silicone), Dr Triana, and others.
If you want to save on money, start checking out medical tourism. Bumrungrad in Thailand is an excellent choice, but there are no doubt great options in the Philippines, and possibly Brazil. Maybe Singapore. Even with the added expense of flight and hotel, it still works out cheaper. The trick will be getting confirmation in advance that one of those surgeons will be willing to do the surgery. Obviously you should be able to get that confirmation in advance, if they're interested in your business.
Speaking of which, there was a woman who posted briefly here a few years back who had done medical tourism. Other than suzy1qtee, that is. I don't recall which Asia Pacific country she went to, but she did indicate she was very pleased with the results. And the cost.
