She's about 5 feet tall, more or less, so breasts of that size are staggering. 
Being about the same height as Chelsea and having perhaps the same generalized size, but not shape?
Chelsea had that polypropylene, aka "Silly String", implant which expanded on its own, but it didn't do it in a good manner which is why her nipples were not straight but off to the side. Part of the reason she had to drain her breasts every so often was due to irregular growth and shape. Chelsea first got that "Silly String" in 1999 or 25 years ago. Demora has better shaped breasts due to advances in breasts implants such as expander implants which are more easily controlled. 
I think that the reason we see nipples migrating to the sides is the placement of the implant in relation to the nipple and subsequent enlargement past the designed intention. Foxy has it too; correctly positioned for 2k implants, the more she expanded, the more they drifted to the sides, the expansion remaining centered on initial placement. Remember post current expanders surgery, how far apart Beshine's boobs were and how they fell towards her sides? They weren't like that at 3k. Apparently it was all due to planning on the future size of her boobs and centering the implants behind the nipples rather than under the pectoral muscle.
In Demora's case, filling with fat allowed her to keep a desired shape because it had nothing to do with the pectoral muscle and her addition of expander implants, which I assume are over the muscle, allows her to maintain a fullness that cannot be maintained by adipose alone over time. I just hope she added a internal bra (maybe the newer, less expensive urethan European version) so she will not experience sagging and so much shoulder strap pain. All in good planning.
