Something really strange on this pic while looking on the huge fingers 
I'm thinking the hand was added to clarify the context more than anything else.
Likely in the original shot, you probably couldn't see her hand at all.
Now, if you look at the angle and position of her wrist, with no hand in site, and your mind is in the gutter, what might it look like she is doing?
That's possibly what they wanted clarify.
