I could be wrong, but I have felt that the rotating nipples are due to a initial medial placement of the implant relative to the nipple, further expansion therefore medial, hence rotation. With Crystal Storm, there may have been a slight lateral placement, thus rotating slightly inward. With Beshine, placement, like so many others, was essentially perfect, however, her internal bra holding the right breast slipped out of place a bit, allowing a downward rotation and producing the ridge on the lateral aspect (slipped to that location rather than remaining against the base as intended) while stressing the medial support (always visible). More argument not to seek under the muscle for really large implants because that point is absolutely going to be medial, which will rotate nipples laterally (see Foxy). It works for smaller implants, but after that point, go over the muscle and hold the implants in place with internal fixation, IMHO.
