Well, my theory is that it has to do with size. The average penis is about 6" but the average fist/forearm is much larger. Since so many people get off about the fantasy of unusual size ( BEA for example ) the one giving the fisting can perhaps fantasize about having a huge penis. Certainly plausible.... then again, it features naked women using their wet vaginas... how is that not going to be of interest, once you get past the obvious shock of it?

Fisting vids are also interesting simply from a novelty/bio-engineering POV (to me at any rate)...
