Yes, on naturally large breasts, some 'spread' often results in a modest degree of outward pointing... this is normal and visually pleasing IMO... the photo above of Danni Ashe is an excellent example.
However, on SOME unusually large breasts (or some breasts that have implants seemingly close to or at the current capacity of the surrounding tissue) it's not uncommon for the nipples to point outwards to a significant/exaggerated degree... though not completely clear in the photo of the blonde above (due to her angle relative to camera) in some cases it's quite extreme. Another, more well-known example of this would be Chelsea Charms. As mentioned, this sometimes happens with un-implanted breasts too, such as Miosotis for example. In some photos of these two hugely busted models (where their breasts are allowed to hang freely) it can seem as if their nipples are practically pointing sideways relative to the model herself.
Nipple placement issues (for implanted breasts) vary in direction and degree of course: Francine Dee's are so far north as to suggest an intentional parody of 'pert' and then of course, there's the aforementioned 'cross-eyed'.
Though I presume that some of this is a byproduct of surgery choices (on the part of the model, or perhaps more commonly, the surgeon him/herself), there's also the fact of the matter that natural breasts aren't always symmetrical to begin with of course. Also possibly involved... perhaps nipples start heading 'outwards' due to excessive 'stretch' in the skin between the nipples: i.e.: if the skin on the inner curve (cleavage side) stretches significantly more than the skin on the outer curve of the breast, it seems like it would be inevitable that the nipple would 'travel' outwards, resulting in that "Marty Feldman" appearance to some degree. Whether this uneven stretch is due to implant placement, or procedure type, or natural physiological variations (or some combination of factors) I don't know. I'm sure someone more versed in implant surgery could elaborate.
