I don't know the story behind that particular shot, but when I worked in the business (1973 or so to 1982) every damnned frame of every damnned skin flick had to be evaluated by the legal department against whatever were the legal standards in each state for "obscenity", and I know that several states (Mostly the Deep South, Utah, and for some bizarre reason, Massachusetts) had a "no female nipples" rule so you couldn't have a mainstream movie that showed nips. In the "adult theaters" you could, of course.

Georgia was a freaking nightmare: Every bloody county had a different standard until the Supreme Court held that a "community standard" had to be at least statewide.

Its current status as an anything-goes liberal bastion notwithstanding, Massachusetts (esp. Boston) was notorious for banning things like Everly Brothers songs into the 1970s.
