As for having your American surgeon go south, I assume that it would be easy to keep news of his performing the surgery  from the FDA -- just don't tell them. But perhaps they have informants everywhere.    

Not so much that they have informants but that there's no statute of limitations on revoking a license for unproffesional conduct.  So, from then on the surgeon has in the back of his head that should anyone who knows about it mention it to the FDA (say, a pissed off colleague or admin type) he/she gets his license yanked.  That's a lot of stress for one surgery.

I've never known of someone who calls him or her self a "cosmetic" surgeon to be respected by other doctors, probably because almost none of them had completed a residency in plastic surgery or were certified in plastic surgery by a legitimate board.
